Who We Are

We provide Community Health Worker services (CHW) with the core goal of promoting well-being, do social prescribing, and train allies.

You do not need to pay for services, we bill your insurance as long as we do social service for you!

CHW services aim to prevent health issues, mental health challenges, and substance use, or stop them from worsening. We promote physical and mental well-being, helping individuals manage all aspects of life for a healthier, longer future.

  • Mental Health Support: Short-term coaching and referrals to county behavioral health services for emotional well-being.

  • Wellness Coaching: Available for college students and youth aged 16–24 on a referral basis. Our coaching focuses on building healthier habits and emotional resilience.

  • Integrative Well-Being Management: Offering alignment exercises, support materials, and strategies for physical and mental balance.

  • Holistic Health Education: Educating on life determinants affecting health such as nutrition, relationships, and work-life balance.

Do you live in Contra Costa County?

Services are open to county residents, with priority for Concord and Pittsburg.

Are you between 18–50 years old?

Minors can qualify only if accompanied by a parent or guardian.

Do you experience anxiety, stress, or trouble sleeping or eating?

Eligibility includes mental or emotional distress related to your current situation.

Has a doctor noted your emotional or mental health concerns?

A healthcare provider's confirmation of stress-related issues can support your eligibility.
