Allyship CHW
Outreach, education, referrals, and navigation of services by compliant Community Health Workers as defined by DHCS.
CHW services are preventive health services designed to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health and well-being. Our trained CHWs help you navigate the healthcare system, connect with resources, and take charge of your health journey.
Eligibility: Available to Contra Costa County residents with qualifying insurance coverage.

Allyship Talk
A community space focused on developing personal agency as a communicator and strengthening the ability to impact one's own health and the health of others.
Participants learn that healthy, honest relationships are essential to emotional well-being, identity, and even physical health. Group sessions are led by a wellness coach, and participants practice 10-minute peer check-ins. Access is based on county districts.
Eligibility: Open to community members who complete self-paced Ally training.

Allyship Wellness Coaching
Supporting individuals in building healthier lives by combining goal-focused wellness guidance with genuine allyship.
Allyship Wellness Coaching provides deep listening, shared advocacy, and compassionate partnership through every challenge. Our coaches work with you to set and achieve wellness goals while providing the support and accountability you need to succeed.
Eligibility: Available for college students and youth aged 16–24 on a referral basis.

Allyship Health Compass
A CHW assessment and eligibility tool that supports care education planning with data analytics and early detection of health barriers.
The Health Compass includes assessment, intervention, and accessibility features. The platform connects CHWs with clients, supports perception-based innovation, and helps with early detection of health barriers. This collaborative tool helps you understand your wellness profile across health dimensions and social determinants of health.
Eligibility: Free to use for anyone. Full services available to eligible residents.

Allyship Health Forum
A community-focused space offering drop-in services where participants can access health education, ask questions, and explore practical tools.
The forum provides guidance on available resources and offers on-the-spot registration and eligibility checks for services. This combines learning, support, and immediate access to care in a collaborative and inclusive environment.
